that are established in the PRIVACY POLICY of Bridgewater
Physicians, P.A. and that are addressed by the Health Insurance
Portability and Privacy Act of 1996.
The Health Insurance Portability
and Accountability Act of 1996 (HIPAA) mandated regulations that
govern privacy, security, and electronic transactions standards
for health care information. These regulations require major
changes in how health care organizations handle all facets of
information management, including reimbursement, coding,
security, and patient records.

complies with established regulations that govern patient
confidentiality and privacy and remains abreast of pending new
regulations as they are proposed and modified. You will be asked
to sign authorizations, consents, and releases in accordance
with our Privacy Policy. We require a copy of the Power of
Attorney document for any patient who has assigned a POA.

office. We urge you to read it carefully. You may request a copy
of the Privacy Policy at registration.
